No idea what that is. It looks like you might have a broken database.
Also, in that second screenshot, I have no idea what that page is. Why did you cut off the URL?

I am using svn 5973.
I am trying to setup an Alert Checker. No matter what I do, I get an error that the alert name must be unique. I have tried strange names in the "Alert Name" field, but everything seems to be taken :-(
Leaving the entity field blank, gives me another error.

I delete them all, and they reappear after a Regenerate.
