The ifDescr part goes into "Entity Match", and after that is saved you need to click the "regenerate alerts" button at the top :)
Make sure it's correctly assocated that alert to that entity.

Your association below needs to be "ifDescr match FastEthernet4/0/3"
You can manually add per-alert checker contacts using the 'alert_contacts' and 'alert_contacts_assoc' tables in the database. For email, the contact endpoint would be 'email'.
The editing UI for this isn't finished yet, and these tables might go away in future if we replace them with something else, but for now you can do it in here.
