Hi,
The 'ports_cbqos' is a memory table.
From Mysql manual:
"The maximum size of MEMORY tables is limited by the max_heap_table_size system variable, which has a default value of 16MB." http://dev.mysql.com/doc/refman/5.6/en/memory-storage-engine.html
You have to raise your 'max_heap_table_size' parameter.

Hi again.
Increased innodb_buffer_pool_size and restarted mysqld – solved the problem.
Buffer_pool_size has probably nothing to do with this issue but I was also getting warnings about
buffer pool is occupied by InnoDB: lock heaps or the adaptive hash index.
It is a good thing to sometimes keep a peek into /var/log/mysqld.log file (or where ever it is stored).
