Ok. I'm not at all sure how it happened, but diffing stable and current showed a single difference:
diff -r html/includes/print/search.inc.php ../dev/html/includes/print/search.inc.php675c675< $fieldset_begin .= '<div class="'.$entry['class'].'">' . PHP_EOL . $fieldset_begin;---> $fieldset_begin = '<div class="'.$entry['class'].'">' . PHP_EOL . $fieldset_begin;
I've corrected this and the forms seem to build properly now!
I guess at some point I edited a file in the /wrong/ tree. Too much Christmas rum, perhaps! :D
So, svn up and it should work again.

Did you already try :
rm -rf htmlsvn up
?
I've just re-synced stable to current, but there are no relevant changes in there. I suspect something to do with CSS, since it looks like the form is hidden under the box-header div. Very odd. Removing html/ and svn up'ing should fix it.

I assume you're both running the stable train? Adam, I think a merge may
be in order.. this works fine on trunk.
