Still the graphs show no data, which is in direct contradiction of the device itself.

Run the poller manually and see if there are any issues, or if it exits early :
./poller.php -h <hostname or device_id>
It should always end with the : "MySQL: Cell[0/0s] Row[8/0s] Rows[33/0.02s] Column[0/0s] Update[72/0.05s] Insert[2/0.02s] Delete[0/0s]" line, else something broke.
It's next to impossible for the ports poller to not work on individual devices, as it's all just IF-MIB, probably the most standard MIB around.
