if this happened on every discovery, you can try debug this as:
./discovery.php -d -m ports,ip-addresses -h <sg300_device>
attach this debug in reply.

I see absolutely no difference between the two IPs...
The IP itself is the one used by the switch, given automatically:
Ipv6 Global Address Type 2a01:ad42:8531:c422:2e36:f8ff:fe5e:49e2/64 Autoconfig
Could it be the autoconfig renew that seems to Observium to be a new IP ?
(Observium Professional edition, Observium 19.8.10001 (stable))
